Physiotherapy through your aged care plan.
Support at Home replaced the Home Care Packages and Short-Term Restorative Care programs on 1 November 2025. Physiotherapy is included among the allied health services that may be available following an aged care assessment and approval.
If you already receive Support at Home, discuss your movement needs with your care partner or provider. We can discuss clinic-based physiotherapy with you and, with your permission, the people coordinating your care.
Start with what matters day to day.
Your physiotherapist can assess the activities you find difficult and discuss a plan suited to your needs. This may involve:
- Walking, balance and steadiness during everyday tasks.
- Strength and movement for getting up from a chair or moving around.
- Exercises suited to your current abilities and goals.
- Reviewing your progress and adjusting the plan.
Read about our strength, balance and mobility care and exercise rehabilitation.
Coordinate your appointment before booking.
- Speak with your care partner. Ask whether physiotherapy is on your approved service list and fits your care plan and available budget.
- Contact our team. We can discuss the clinic appointment, referral information and invoicing arrangements with you or your authorised care contact.
- Confirm the arrangements. Your registered aged care provider needs to agree to the service and invoicing arrangements before your first visit.
An online appointment does not establish approval to use your aged care funding.
How physiotherapy funding works.
Physiotherapy is a clinical support under Support at Home. There is no participant contribution for eligible physiotherapy funded through the program, but the service still draws on your available care budget. Your approved services, care plan and provider arrangements determine what can be funded.
Tell your care partner if you have an existing GP care plan or another healthcare funding arrangement. Existing eligible services under those arrangements should be used first, where required, before Support at Home funds are used.
Getting ready for your first visit.
Bring relevant referral information, recent reports, current exercise advice and any mobility aid you normally use. Think about activities you want to maintain or work towards. A family member or support person can join the discussion if you choose.
Our appointments are inside Elizabeth Park Healthcare, 1 Gould Rd. See our clinic and waiting-area information, and tell us about communication or access needs before you arrive.
Helpful questions.
Can my care partner organise the appointment?
Yes, your care partner or authorised contact can call to discuss the service and the arrangements needed. We will involve you in decisions about your care and confirm your consent before sharing relevant information.
Are these home-visit physiotherapy appointments?
The appointments described here take place at our Elizabeth Park clinic. Call to discuss whether a clinic appointment suits your needs before choosing a time.
What if I have not been assessed for aged care services?
My Aged Care explains how to request an assessment and who may be eligible. Our team can discuss a private clinic appointment, but cannot approve Support at Home funding.
