What changed

From 1 July 2025, new plans are made under the GP Chronic Condition Management Plan (GPCCMP) framework. You may still hear the older terms EPC, CDM, GP Management Plan (GPMP) or Team Care Arrangement (TCA).

Eligible GPMPs and TCAs made before 1 July 2025 can support services until 30 June 2027, but a valid referral is still needed.

Who decides if I am eligible?

Your GP. Plans are for people with a chronic or terminal medical condition, and your GP decides whether a plan and a physiotherapy referral are appropriate for you.

How many visits?

The usual limit is up to five individual allied health services per calendar year, shared across eligible professions. Podiatry, dietetics or other allied health visits use the same allowance. Eligible Aboriginal and Torres Strait Islander patients may access up to ten services under the relevant arrangements.

Is it free?

Not automatically. A Medicare rebate applies to eligible services, and there may be an out-of-pocket amount. Ask our team about the charge and claiming arrangements before your appointment. See fees and rebates.

What to bring

  • Your Medicare card.
  • Your GP referral for physiotherapy.
  • Any relevant plan information and reports.
  • How many allied health visits you have already used this year.

After your first and last service under the referral, your physiotherapist sends a report to your GP.
